Architecturally: the only code path that can touch a document begins at an approved proposal.

Every change is reversible

Full pre-edit snapshot before any write. If someone edited the same text since, revert refuses rather than clobbering their work.

Edits are re-verified against the live document at the moment of approval. Uncertain proposals are dropped before anyone sees them.
